PHI and this website
This website is not a portal for protected health information (PHI). Do not send detailed medical records, dates of birth, or diagnostic details through the contact form. If you are an existing patient, use the patient portal or contact the office directly.

- With service providers who help operate the website (hosting, analytics, form handling), under written agreements limiting their use of information to providing those services.
- When required by law.
- Google Ads receives advertising measurement information as described below. We do not send form contents or customer matching identifiers to advertising platforms.
Cookies and your choices
A regional banner offers equal advertising acceptance and rejection choices where consent is required. If your location cannot be determined, advertising remains off until you accept. Elsewhere, measurement is permitted by default unless you refuse or use a recognized browser opt-out, including Global Privacy Control or Do Not Track. Advertising and analytics choices are separate.
Google Ads measures successful inquiry submissions, not Book Now clicks or completed appointments. When advertising is permitted, a Google ad click identifier may be kept for this browser session and included to attribute an inquiry to an ad. Arbitrary query strings and referring addresses are not passed in event parameters. The event contains no name, email, phone, inquiry topic, message, monetary value or treatment-specific information. Automatic page views, remarketing, advertising personalization and enhanced conversions are disabled. Meta Pixel and customer matching are not enabled.
If you arrive from one of our ads (for example on Facebook or Instagram), we may note the ad source and campaign name from the link for this browser session and save them with your inquiry, so we know which ads lead to messages. This stays on our own systems and is not shared with Meta or other advertising platforms; ad click identifiers are not stored.
Your choices are stored on this browser with a random visitor reference, timestamp, region, notice version and the text and choices presented. Earlier decisions are preserved when you withdraw. Open Cookie preferences at any time to change your choice; opening it does not reset your current decision. Withdrawal applies immediately and across other open tabs. Events blocked when they occurred are never replayed after acceptance. Clearing browser storage removes this browser's preference record.
